The Incedo platform is designed with a library of assessment and screening tools that autogenerate patient-centric care plans that are further personalized through Care Manager interactions with the patient to identify their specific concerns and goals. Workflows configured for your organization help propel Care Managers through the tasks and interventions available through your ACO. Our CareConnect portal allows Care Coordinators to communicate with patients remotely and monitor progress to intervene before in-person care is needed.
