The Annual Wellness Visit (AWV): The AWV is a little-known benefit and free service to Medicare patients and CCM is covered by most insurance plans. Chronic Care Management (CCM): As of January 1, 2015, Medicare began reimbursing for Chronic Care Management (CCM) services using CPT Code 99490. This service is for Medicare patients with multiple chronic conditions and is non-face-to-face. It pays for services like phone calls, coordinating with other providers, and checking on medications. Until recently, these services have always been done by providers, but they were never permitted to bill for them.
But although providers can now can get paid for CCM, the practices must meet certain CCM billing requirements which are strict and very specific.
